背景 注意偏向矫正(ABM)是近年来情绪调节领域研究的热点之一,指通过计算机程序反复训练个体对中性或正性刺激的关注以达到纠正其对负性刺激的注意偏向、改善异常认知的过程。近年来,国内外公开发表的ABM治疗抑郁的文献报道逐年增多,但评价指标较为分散,相关循证证据不足。 目的 系统评价ABM疗法对抑郁患者的干预效果。 方法 计算机检索PubMed、The Cochrane Library、EMBase、中国生物医学文献数据库(CBMdisc)、中国知网(CNKI)、万方数据知识服务平台和维普网(VIP),搜集有关ABM治疗抑郁患者的随机对照试验(RCT),检索时限均为建库至2021-12-31。干预措施:试验组采用基于计算机的ABM治疗;对照组实施同等条件下的安慰训练,包括空白对照、假ABM刺激等。主要结局指标包括汉密尔顿抑郁评定量表(HAMD/HRSD)、贝克抑郁量表(BDI)和流行病学研究中心抑郁量表(CES-D)评分;次要结局指标包括状态-特质焦虑量表(STAI)、冗思反应量表(RRS)评分。由两位研究者独立筛选文献、提取资料,并采用Cochrane提供的偏倚风险评估工具2.0版本(RoB 2.0)进行偏倚风险评价,采用RevMan 5.4和Stata 12.0软件进行Meta分析。 结果 最终纳入13篇文献,其中1篇文献提取为2个试验报告,共包含968例患者。13篇文献均为RCT,其中7篇文献评价为低偏倚风险,4篇文献评价为中偏倚风险,2篇文献评价为高偏倚风险。Meta分析结果显示,试验组抑郁水平、焦虑情绪及冗思反应的改善优于对照组(P≤0.05);亚组分析:干预后随访时间<2个月时,试验组与对照组BDI-Ⅱ、HAMD评分比较,差异无统计学意义(P>0.05);干预后随访时间≥2个月时,试验组与对照组BDI-Ⅱ评分比较,差异无统计学意义(P>0.05)。 结论 ABM疗法能明显改善抑郁患者的抑郁、焦虑及冗思反应,但该疗法的长期疗效仍需进一步研究。  相似文献

Depressive disorder is a complex, heterogeneous disease that affects approximately 280 million people worldwide. Environmental, genetic, and neurobiological factors contribute to the depressive state. Since the nervous system is susceptible to shifts in activity of epigenetic modifiers, these allow for significant plasticity and response to rapid changes in the environment. Among the most studied epigenetic modifications in depressive disorder is DNA methylation, with findings centered on the brain-derived neurotrophic factor gene, the glucocorticoid receptor gene, and the serotonin transporter gene. In order to identify biomarkers that would be useful in clinical settings, for diagnosis and for treatment response, further research on antidepressants and alterations they cause in the epigenetic landscape throughout the genome is needed. Studies on cornerstone antidepressants, such as selective serotonin reuptake inhibitors, selective serotonin and norepinephrine reuptake inhibitors, norepinephrine, and dopamine reuptake inhibitors and their effects on depressive disorder are available, but systematic conclusions on their effects are still hard to draw due to the highly heterogeneous nature of the studies. In addition, two novel drugs, ketamine and esketamine, are being investigated particularly in association with treatment of resistant depression, which is one of the hot topics of contemporary research and the field of precision psychiatry.  相似文献

Breast cancer is believed to be driven by epigenetic regulation of genes implicated in cell proliferation, survival, and differentiation. Recently, aberrant N6-methyladenosine (m6A) decorations turned up as crucial epigenetic regulator for malignant breast cancer, which may serve as new targets for breast cancer treatment. Here we briefly outline the functions of m6A and its regulatory proteins, including m6A “writers,” “readers,” and “erasers” on RNA life fate, recapitulate the latest breakthroughs in understanding m6A modification and its regulatory proteins, and the underlying molecular mechanisms that contribute to the carcinogenesis and the progression of breast cancer, so as to provide potential epigenetic targets for diagnosis, treatment and prognosis in breast cancer.  相似文献

IntroductionThe risk factors of breast cancer overlap with those of peripheral arterial disease (PAD), with increasing prevalence. In addition, there is under-utilization of risk factor modification measures in patients with PAD.Materials and MethodsElectronic medical records of patients with breast cancer with International Classification of Diseases 9/10 codes for PAD spanning 10 years from June 1, 2009 to June 1, 2019 were reviewed.ResultsA total of 248 patients, 98% women, with a median age of 75 years and with a median follow-up of 76 months, were included. PAD risk factors were identified as smoking (44%), obesity (38%), hyperlipidemia (68%), hypertension (HTN) (74%), and diabetes (42%). Overall, survival was significantly impacted by smoking (P = .0301) and HTN (P = .0052). In a Cox proportion hazard ratio regression, HTN (overall death hazard ratio [HR], 3.1784; 95% CI, 1.0291-6.7490; P = .0070; cancer-related death HR, 2.6354; 95% confidence interval [CI], 1.0291-6.7490; P = .0434) and smoking (overall death HR, 1.7452; 95% CI 1.0707-2.8444; P = .0255; cancer-related death HR, 2.7432; 95% CI, 1.4190-5.3030; P = .0027) were predictors of overall death and cancer-related death. Of all patients, 48% were on statins and 54% were on antiplatelet therapies. Of the patients, 62% of current smokers were offered a smoking cessation program, 27% of obese patients were offered a nutrition consult, 42% of patients with diabetes had blood glucose controlled, and 54% of patients with HTN had blood pressure controlled.ConclusionSmoking and HTN are risk factors associated with decreased survival and predictive of overall death and cancer-related death. Frequent consumption of acidic beverages is related to excessive tooth wear, namely dental erosion. Preventive measures may involve reduction or elimination of acidic drink consumption. However, the success of this approach is difficult to achieve as it is highly dependent on patient compliance. Therefore, a practical way of minimizing the erosive potential of popular acidic drinks may be their chemical modification. The aim of this article was to review the different methods of modification and their shortcomings. The available literature demonstrates that the erosive potential of most acidic beverages could be reduced. To date, the effectiveness of soluble calcium salts supplementation is the best established. However, modification can reduce the sensorial quality of the drink and shorten its shelf‐life. There is also a need to evaluate the lowest effective and safe dose of the additive.  相似文献
